Please complete all items below to register for a CHL Class or CHL Renewal Class.  Prior to completing this form, please review the CHL Class page of our web site for more information and class schedules.

Cost for this class for New CHL License Applicants is $100.00 for Current BASC Members and $120.00 for Non-Members.

Cost for the class for CHL License Renewal Applicants is $80.00 for Current BASC Members and $100.00 for Non-Members.
